This procedure explains various ways to Apply For Social Security System (SSS) Membership in CALABARZON.

Apply OnlineEdit

  1. Here's how you can apply and be a member of Social Security System (SSS). The easiest way is to register online through SSS's website. To start your online application, click this link – Register as an SSS member. When the page opens click the word "here" (written in blue).
  2. On the next page, a step by step guide will be shown along with some reminders. After reading it, if you think you are ready to process your application click the “Start” button which will take you to the Registrant Record Verification Form. Fill out the form.
  3. After you have completed the form, tick the certification button and then submit your application by clicking the “Submit” button.
  4. A confirmation link will be sent to the e-mail address that you have provided on your application.
  5. Open the email sent to you by SSS and click the link to continue with your online SSS number application.
  6. The link will redirect you back to SSS’s website where you will be asked to complete the basic information and contact form.
  7. After completing the basic and contact information form, indicate the reason why you need a SSS number by ticking the appropriate button (under Purpose for Registration). Once you are done with the form, click the “Next” button.
  8. On the next page, provide information about your social status and details about your birth. When you are done with the form, click the “Next” button.
  9. Next, you will need to provide information about your benificiary(ies). Click the “Next” button when you are done.
  10. After completing all of the online forms, you will be taken to the summary page which contains all the information that you have provided in your application. Review the form and make sure that all the entries are correct.
  11. For the last step, click the “Generate SSS Number” button to receive your SSS number.
  12. The system will generate and show you your SSS number. Print your e-personal record (you will need to submit this later on) and your SSS number slip.
  13. Also, you will receive an email confirmation which contains your SSS number and instructions on how you can complete your SSS number application.
  14. To complete your registration, you will need to go to any SSS branch and submit your printed e-personal record together with all the required documents.   15. After you have finalized your registration, you may now start using your newly issued SS number and began paying your monthly contribution.

Apply In PersonEdit

  1. You may also apply to be a member of SSS in person by going to any SSS branch.
  2. Complete a Personal Record (SS Form E-1). A link is provided under 'Documents to Use' section which you can click to download the form.
  3. Find the counter that process new member applications and submit your completed form together with all the required documents.
  4. The SSS officer will encode your information and issue your SS number.
  5. You may now start using your newly issued SS number and began paying your monthly contribution.

    • Health or Medical Card
    • Home Development Mutual Fund (Pag-IBIG) Member's Data Form
    • ID card issued by (LGUs) eg: Barangay/Municipality/City
    • ID card issued by professional association recognized by PRC
    • Life Insurance Policy
    • Marriage Contract / Certificate of Marriage
    • Membership card issued by private companies
    • National Bureau of Investigation (NBI) Clearance
    • Overseas Worker Welfare Administration (OWWA) Card
    • Philippine Health Insurance Corporation (PHIC) ID card / Member Data Record
