This procedure explains various ways to Obtain a Social Security System card in Bacolod.

Apply In-PersonEdit

  1. Following are procedures on how you can apply for a Social Security System Card or UMID (Unified Multi-Purpose ID) Card. In case you are confused, the following government agencies: SSS, GSIS, PhilHealth and Pag-IBIG now issues UMID cards. Instead of asking members to apply for four different ID cards from each of those agencies, they decided to just issue one card which members can then use in transacting with the agencies mentioned. So, when you apply for a SSS Card the form that you would be asked to complete is a Unified Multi-Purpose ID (UMID) Card Application. The form is available through SSS's website. A link is provided under 'Documents to Use' section which you can click to download the form. You can also ask for a copy of the form from any SSS branch that process UMID applications.
  2. When you fill out the form make sure to write your correct and complete mailing address. For the "Type of card application" item, tick the 'INITIAL ENROLLMENT' Box. You may also request for a UMID Card that can be used as an ATM Card as well. For this you will also need to tick the 'UMID Card as ATM Card' box and provide the name of the bank and the branch.
  3. Please note that not all SSS branches process UMID applications. Before you visit a SSS branch inquire first (online or via phone) if they have UMID Card Enrollment Facility in their branch. You can also check out this link - SSS Branches with UMID Enrollment Facilities.
  4. At the SSS office, submit your completed application form and present a valid ID.
  5. After reviewing your documents, the SSS representative will ask you to go to the booth where they will take your photo, scan your fingerprints, and save your digital signature. You may now go home after this process.
  6. Wait for the SMS notification form SSS informing you that your UMID card is now ready for pick up.
  7. Collecting your card:
    • If you applied for a UMID Card as ATM card, you need to claim your card at the bank branch you wrote in your application. You must claim your card within thirty (30) days upon receipt of SMS notification from SSS.
    • For regular UMID card, claim your card at the SSS branch where you applied. You must claim your card within sixty (60) days upon receipt of SMS notification from SSS.

    • Certificate of Naturalization from the Bureau of Immigration
    • Company ID card
    • Company Representative Authorization Card (ACR) issued by SSS
    • Credit card
    • Firearm License card issued by Philippine National Police (PNP)
    • Fisherman's card issued by Bureau of Fisheries and Aquatic Resources (BFAR).
    • Health or Medical Card
    • Home Development Mutual Fund (Pag-IBIG) Member's Data Form
    • ID card issued by (LGUs) eg: Barangay/Municipality/City
    • ID card issued by professional association recognized by PRC
    • Life Insurance Policy
    • Marriage Contract / Certificate of Marriage
    • Membership card issued by private companies
    • National Bureau of Investigation (NBI) Clearance
    • Overseas Worker Welfare Administration (OWWA) Card
    • Philippine Health Insurance Corporation (PHIC) ID card / Member Data Record
    • Police Clearance
    • Postal ID Card
    • School ID Card
    • Seafarer's Registration Certificate issued by (POEA)
    • Senior Citizen card
    • Student Permit issued by Land Transportation Office (LTO)
    • Taxpayer's Identification Number (TIN) Card
    • Transcript of Records
    • Voter's Identification card or Affidavit / Certificate of Registration

EligibilityEdit

  • No existing SSS ID or UMID card
  • Paid at least one-month SSS contribution
  • Valid date of coverage (the date you started as an active member per SSS records)

FeesEdit

SSS / UMID cards are free of charge for first-time applicants (those who have not been issued a SSS ID or UMID card before).

ValidityEdit

The SSS or UMID Card has no expiry date.

Note:all SSS branches can process SSS Card or UMID card applications. Before you visit a SSS branch inquire first (online or via phone) if they have UMID Card Enrollment Facility. Also, you need to apply for your card personally, you cannot ask a representative to apply for you because the process includes taking the applicant's photo, saving his/her digital signature and scanning his/her fingerprints.
