Apply In-Person:

1. To Apply for Health Certificate for Animals, the applicant must visit the Veterinary Hospital of their respective area.
2. Collect the applicable application form from the Veterinary Hospital office.
3. Fill the application form with the relevant information.
4. Attach the required documents as per the “Required Documents” section below.
5. Submit the application form to the office.
6. Payment will have to be made as per the requirement of the Office. Additional fees may be charged for the process.
7. After submission, the applicant has to wait for 2-3 days to receive the Health Certificate.

• Animal health certificate only applies to those animals whose country of origin is India.
  
• Must be a licensed owner of an Animal with Pin number
  
• Must have a Vaccination card (against any diseases) of that particular animal.

The Certificate is valid for Four months
