Procedure

Apply In-person:

1. The applicant willing to apply for health certificate for animals must visit the Animal Husbandry and Veterinary Department of Assam and submit an application to the Animal Husbandry and Veterinary Department official.
2. The applicant can download the certificate from the link below: link
3. The applicant must fill the relevant details of the animal including description of the animal and vaccination of the animal.
4. The Animal Husbandry and Veterinary Department official verifes the application form.
5. Once the application form is verified, the Health Certificate for animals in provided to the applicant.

Required Documents

The following are the required documents to apply for this certificate.

• Application Form
• Passport size photos of the Animal (2 recent photos)
• Xerox vaccination card (Vaccinated for any disease)
• Xerox copy of license register for animal
• Laboratory Reports (not mandatory in some cases)
• Original Health documents are compulsory
• Country of origin
• Adhar card of the owner
• Passport size photos of the owner
• Residential proof of the owner
  • Water Bill
  • Telephone (landline or post-paid mobile bill)
  • Electricity bill
  • Income Tax Assessment Order
  • Election Commission Photo ID card
  • Proof of Gas Connection
  • Certificate from Employer of reputed companies on letterhead
  • Spouse’s passport copy
  • Parent’s passport copy, in case of minors(First and last page)
  • Aadhar card
  • Rent Agreement
  • Photo Passbook of running Bank Account

Eligibility

• Animal health certificate only applies to those animals whose country of origin is India.
• Must be a licensed owner of an Animal with Pin number
• Must have a Vaccination card (against any diseases) of that particular animal.

Fees

• Payment will be made as per the requirement of the Office.
• Additional fees may be charged for the process.

Validity

• The Certificate is valid for Four months
