Procedure

Apply In-person:

1. To Apply for Health Certificate for Animals, the applicant must visit the Veterinary Hospital of their respective area. The address and contact details of all the Veterinary Hospitals of Nagaland can be found in this Contact Link
2. Collect the applicable application form from the Veterinary Hospital office.
3. Fill the application form with the relevant information.
4. Attach the required documents as per the “Required Documents” section below.
5. Submit the application form to the office and make the required payment.
6. After submission, the applicant has to wait for 2-3 days to receive the Health Certificate.

Required Documents

The following are the required documents to apply for this certificate.

• Application Form
• Passport size photos of the Animal (2 recent photos)
• Xerox vaccination card (Vaccinated for any disease)
• Xerox copy of license register for animal
• Laboratory Reports (not mandatory in some cases)
• Original Health documents are compulsory
• Country of origin
• Adhar card of the owner
• Passport size photos of the owner
• Residential proof of the owner
  • Water Bill
  • Telephone (landline or postpaid mobile bill)
  • Electricity bill
  • Income Tax Assessment Order
  • Election Commission Photo ID card
  • Proof of Gas Connection
  • Certificate from Employer of reputed companies on letterhead
  • Spouse’s passport copy
  • Parent’s passport copy, in case of minors(First and last page)
  • Aadhar card
  • Rent Agreement
  • Photo Passbook of running Bank Account

Eligibility

• Animal health certificate only applies to those animals whose country of origin is India.
• Must be a licensed owner of an Animal with Pin number
• Must have a Vaccination card (against any diseases) of that particular animal.

Fees

• Payment will be made as per the requirement of the Office.
• Additional fees may be charged for the process.

Validity

• The Certificate is valid for Four months
