This procedure explains various ways to Replace a Lost or Stolen or Damaged SSS Card in SOCCSKSARGEN.

Apply In-PersonEdit

  1. Following are procedures on how you can replace your lost, stolen, or damaged SSS card. When you apply to replace your old SSS Card, what you will actually get is the new Unified Multi-Purpose ID (UMID) Card.
  2. To apply, go to any SSS Branch and fill out the Unified Multi-Purpose ID (UMID) Card Application form. The form is available through SSS's website. A link is provided under 'Documents to Use' section which you can click to download the form. You can also ask for a copy of the form at the SSS branch. Please note that not all SSS branches process UMID applications. Before you visit a SSS branch inquire first (online or via phone) if they have UMID Card Enrollment Facility in their branch.   3. When you fill out the form make sure to write your correct and complete home address. For the "Type of card application" item, tick either the 'Replacement of Lost Card' or 'Replacement of Damaged Card' box. You may also request for a UMID Card that can be used as an ATM Card as well. For this you will also need to tick the 'UMID Card as ATM Card' box and provide the name of the bank and the branch.
  4. At the SSS office, submit your completed application form together with all the additional requirements. If you have lost your SSS card you will also be asked to submit a notarized Affidavit of Loss. You can have one made with the help of a notary public. For damaged cards, you may also be asked to surrender your old damaged card.
  5. Pay the replacement fee. Don't forget to collect your Official Receipt (proof of payment).
  6. After reviewing your documents, the SSS representative will ask you to go to the booth where they will take your photo, scan your fingerprints, and save your digital signature. You may now go home after this process.
  7. Wait for the SMS notification form SSS informing you that your UMID card is now ready for pick up.
  8. Collecting your card:
    • If you applied for a UMID Card as ATM card, you need to claim your card at the bank branch you wrote in your application. You must claim your card within thirty (30) days upon receipt of SMS notification from SSS.
    • For regular UMID card, claim your card at the SSS branch where you applied. You must claim your card within sixty (60) days upon receipt of SMS notification from SSS.

Primary ID card (any one of the following, present original):

Any two (2) other ID cards/documents, both with signature and at least one (1) with photo (if the applicant don't have any of the primary cards mentioned above)


Additional requirements:

  1. For card replacement due to amendment of data/authenticating finger:
    • Previously issued SS digitized ID or UMID card of the card applicant
    • Proof of payment
  2. For card replacement due to lost of SS digitized ID or UMID card
    • Duly notarized Affidavit of Loss
    • Proof of payment
  3. For card replacement due to non-receipt of UMID Card
    • Duly notarized Affidavit of Non-Receipt of Card
    • Notice/Email from Identity Management Department (IMD) that the courier lost/was not able to deliver the UMID card
    • Proof of payment
  4. For card replacement due to damaged UMID Card, UMID Card as ATM Card and other reason/s:
    • Proof of payment

  • Not all SSS branches can process SSS Card or UMID card applications. Before you visit a SSS branch inquire first (online or via phone) if they have UMID Card Enrollment Facility. Also, you need to apply for your card personally, you cannot ask a representative to apply for you because the process includes taking the applicant's photo, saving his/her digital signature and scanning his/her fingerprints.
