Procedure

This procedure explains various ways to Register a Kasambahay With SSS, PhilHealth and Pag-IBIG in Muntinlupa.

Apply In Person

1. Here's how you can register your Kasambahay with SSS, PhilHealth and Pag-IBIG. Since you will be using the Unified Registration Forms you do not need to submit separate applications to SSS, PhilHealth and Pag-IBIG, you can submit your application (just one) to the any nearest Pag‐IBIG/PhilHealth/SSS branch. But, please note that after registration, if you have any update/s or change/s in the Employer Information you should file a separate application to each of the 3 Agencies - Pag-IBIG, PhilHealth and SSS.
2. Registration of the Kasambahay is the household employer's responsibility. The application can be submitted/filed personally by the household employer or by his/her authorized representative. When you go to a Pag‐IBIG/PhilHealth/SSS branch you will be asked to complete a Household Employer Unified Registration Form (to be completed by the household employer or his/her representative) and Kasambahay Unified Registration Form (to be completed by the kasambahay). The forms are available at any Pag‐IBIG/PhilHealth/SSS branch/One Stop Shops. You can also download the forms online, links are available under "Documents to Use" section.
3. Submit the completed forms.
4. Pag‐IBIG/PhilHealth/SSS branch will review and process the registration and issue the membership numbers.
5. For the SSS, if the Registrant is unavailable to sign the document, SS Number shall still be issued, subject to the following conditions:
   • Membership Status of Kasambahay is "Temporary".
   • The SS Number issued can only be used for contribution payment and employee reporting (by the Household Employer).
   • Submission of Primary or Secondary document/s and/or signature in the Form is required for conversion of Membership Status to Permanent, thru Member's Data Amendment Form (SSS Form E‐4)
   • Availment of SSS Benefits and Loans is only allowed for Permanent Membership Status, subject to qualifying conditions.

Notes:

1. If the application is filed/submitted personally by the Household Employer, no supporting document is required to be submitted.
2. If the application is filed/submitted through an Authorized Representative of the Household Employer, presentation of the following is required:
   • Letter of Authorization from Household Employer
   • Valid ID of the Household Employer
   • Valid ID of the Authorized Representative

Notes:

1. The form is not applicable for Family Driver. Registration of Family Driver must be filed separately in each agency (Pag‐IBIG/PhilHealth/SSS).
2. Documentary requirement is not yet needed at the time of registration for the issuance of Pag‐IBIG/PhilHealth/SSS Numbers. You will be asked to present documentary requirements when you start to avail of the benefits this is to prove the member's identity and payment of required premium contributions.
3. List of Accepted Documents (this is for SSS only):
   1. Primary Documents: Submit photocopy & present original/certified true copy of any of the ff:
      • Birth Certificate
      • Baptismal Certificate
      • Drivers License
      • Passport
      • Professional Regulation Commission (PRC) Card
      • Seaman's Book
   2. Secondary Documents: In the absence of Primary Documents, submit photo copy and present original/certified true copy of TWO (2) of the following, BOTH should bear the name and at least ONE (1) should indicate the Date of Birth:
      • ATM Card
      • Bank Account Passbook
      • Birth/Baptismal Certificate of Children
      • Marriage Contract
      • NBI Clearance
      • Police Clearance
      • Postal ID Card
      • Voter's ID/Affidavit
      • School Records
      • ID Card issued by Local Government Units (e.g. Barangay, Municipal/City)

Eligibility

• A Kasambahay or a House Helper is any person who renders domestic or household services and receives compensation for such services.
• A Kasambahay or a House Helper is required under the law to be registered with SSS, PhilHealth and Pag-IBIG if the kasambahay is 60 years old and below and receiving a monthly compensation of at least P1,000.00

As to who should register the kasambahay:

• Registration of the Kasambahay is the household employer's responsibility. A household employer is any person who uses the services of a house helper. As a rule, the husband or wife or any other person who actually pays the compensation for such services shall be deemed the household employer.

Fees
