Procedure

Apply In-Person:

1. Applicant shall approach their respective municipalities/the department office from where they obtained birth certificate.
2. Obtain 'Birth Certificate Update/correction Form' from the respective counter or write in a paper to apply.
3. Please complete the application form and attach the required documents and submit it to the receiving authority.
4. The accepted application and documents will be verified by the authority and an acknowledgement will be provided for applying.
5. Collect the acknowledgment slip after submitting the form and keep it safe for future reference.
6. This application will be processed further by the respective department.
7. Once the process is over, applicant gets notification to collect the certificate.
8. Use the acknowledgment slip to obtain certificate from the office as per notified period.

Note: This changes will be published in a local newspaper and with the respective state official Gazette publication.

• In some states applicant shall approach over the counter services like CSC centres/e-seva etc to apply.
• In some states applicant shall apply using the respective state department online portal.

• Affidavit on a non-judicial stamp paper of minimal about the reason of name change.
• Address proof (Voter ID, Driving License etc.) of parents as per requirement
