1. If you have lost your NSSF card, go to the nearest police station to report this.
2. Obtain the police report stating you loss.
3. Visit the nearest NSSF office in your region
4. Present the police report to the attending officer, with your identification document.
5. Answer the question that the officer will ask you and fill the application from the they will provide you
6. You biometric data will be taken again and they will start processing your application
7. After processing your card, it will be issued to you immediately.
8. If you want to replace a damaged NSSF card, present it for replacements with your identification documents.

• Report your loss to the police station, obtain the police report explaining your loss and visit the nearest NSSF office. Ask them for assistance, you will be provided with the application form fill it and your biometric data will be taken, the application will be processed and a new card will be issued to you within 15 minutes.
