- To Get Birth Affidavit, the applicant has to approach the U.S Department of States link
- The applicant has to download the application form from the given following link
- The applicant has to enter all the necessary details in the application form.
- Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
- The applicant has to attach the photocopies of the required documents along with the application form while submitting.
- Submit the form and the required documents to the Court, State or other institution that helps verify a person’s birth information and can be used if the birth record is incomplete.
- If the applicant is applying for passport needs to submit the passport form along with the affidavit form.

- Fee for this procedure has to be paid as per the authority’s request.
- Once all the verification is done by the authority, the applicant receives a notification through a mail.
- The applicant has to download the certificate from the mail.
- The processing time for this procedure is up to 30 days.

Need for the Document
- The affidavit of birth certificate is used to verify under oath the facts that surround a person’s birth. In cases when a person cannot provide a birth certificate, they may submit an “Affidavit of Birth Certificate” as a substitute.
