Procedure

Apply In Person

1. To Get Birth Affidavit, the applicant has to approach the U.S Department of States link
2. The applicant has to download the application form from the given following link
3. The applicant has to enter all the necessary details in the application form.
4. Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
6. Submit the form and the required documents to the Court, State or other institution that helps verify a person’s birth information and can be used if the birth record is incomplete.
7. If the applicant is applying for passport needs to submit the passport form along with the affidavit form.

9. Fee for this procedure has to be paid as per the authority’s request.
10. Once all the verification is done by the authority, the applicant receives a notification through a mail.
11. The applicant has to download the certificate from the mail.
