Procedure

Apply In-Person:

1. In order to change the family head name in the ration card, the applicant shall approach their respective ration office or circle office or GRC centers in their respective area as applicable.
2. Go to the respective office and collect the application form or write on a plain A4 sheet as advised.
3. Application form can be downloaded using the following link
4. Complete the application form. Attach the required documents as per our “Required Documents” section of this page and submit it to the designated receiving authority.
5. Authorities after checking the submitted application along with attached documents will make entries and provide you record number for reference.
6. Keep it safe for future reference.
7. This application will be processed further to issue the ration card with updated family head name as per notified period.
8. The ration card will be delivered as per applicable norms.

• The validity remains the same.
  
• Usually Family identity card is valid for lifetime and will be renewed in 5 years or based on announcement.

• Do not submit fake documents as a proof to change the family head name.
  
• In case the mobile number associated with your card is not in service, then apply to replace the mobile number.

Need for the Document

• For implementation of the Targeted Public Distribution System(TPDS), Ration Card is being issued to all the heads of families, with details about family members included in the card. In some states, the government grants financial sanctions and other benefits to the woman family head as per ration card. To avail those benefits, the family head in the ration card need to be changed from respective husband to wife.
