Procedure

Apply In-Person:

1. In order to change the family head name in the ration card, the applicant shall approach their respective ration office Or Food & Civil Supplies officials in urban and Municipal/ Town Committee Members in rural areas in their respective area as applicable.
2. Link for the office contact details link in the given page, please select “ABOUT NAGALAND” menu and choose “DISTRICTS” option to get a new page. please select the “Visit Website” button below the respective district name as appropriate. User will reach respective district portal. In the given page, select menu option “Cont. Us” or “Contacts” or “Contact Us” menu for respective contact details. Use the contact details to find out the office information for your requirement.
3. Go to the respective office and collect the application form or write on a plain A4 sheet as advised.
4. Complete the application form. Attach the required documents as per our “Required Documents” section of this page and submit it to the designated receiving authority.
5. Authorities after checking the submitted application along with attached documents will make entries and provide you record number for reference.
6. Keep it safe for future reference.
7. This application will be processed further to issue the ration card with updated family head name as per notified period.
8. The ration card will be delivered as per applicable norms.

Need for the Document

• For implementation of the Targeted Public Distribution System(TPDS), Ration Card is being issued to all the heads of families, with details about family members included in the card. In some states, the government grants financial sanctions and other benefits to the woman family head as per ration card. To avail those benefits, the family head in the ration card need to be changed from respective husband to wife.
