Procedure

Apply In-Person:

1. In order to change the family head name in the ration card, the applicant shall approach their respective ration office or the mamlatdar office /Jan seva Kendra in their respective area as applicable.

Then select your district from the dropdown list against “Select a district” and your taluk from the dropdown list against “Select taluka”. Once the area selection is over, please enter the captcha as appropriate and click “Search” button AND link1 in the given page, please select the website hyperlink as per your choice to reach respective district website. Please select “Offices > Mamlatdar Office” menu to get mamlatdar contact details and “HOME > SERVICES > Digital Gujarat” or “DEPARTMENTS > COLLECTORATE-REVENUE > Registry (Jan Seva)” for Jan seva Kendra at respective municipality.
3. Go to the respective office and collect the application form or write on a plain A4 sheet as advised.
4. Complete the application form. Attach the required documents as per our “Required Documents” section of this page and submit it to the designated receiving authority.
5. Authorities after checking the submitted application along with attached documents will make entries and provide you record number for reference.
6. Keep it safe for future reference.
7. This application will be processed further to issue the ration card with updated family head name as per notified period.
8. The ration card will be delivered as per applicable norms.

Need for the Document

• For implementation of the Targeted Public Distribution System(TPDS), Ration Card is being issued to all the heads of families, with details about family members included in the card. In some states, the government grants financial sanctions and other benefits to the woman family head as per ration card. To avail those benefits, the family head in the ration card need to be changed from respective husband to wife.
