Procedure

• Visit the concerned authority.
• Fill the application form with the complete information.
• Submit all the necessary document.
• Pay the prescribed fee.

Required Documents

• A copy of your passport
• Original and copy of proof of identity
• Original and copy of the driver's license(personal vehicle or lifetime)
• Recent photograph
• Proof of the name - surname, marriage or divorce.

If cannot be done in person, additional requirements must be submitted.

• Authorization letter stamp duty 10 baht
• A copy of the ID card of the appointee
