Odisha - Online Correct or Update a Death Certificates

Step 2 :Please use the following link to apply online : Apply Online.

Step 3 : In the given page, click on the “Death” button which is highlighted in “Red” in the below image.

Step 4 : On the next page, Select the district under the heading “Choose your district” from the drop down menu which is highlighted in “Red” in the below image.

Step 5 : Then, Select the ULB under the heading “Choose your ULB” from the drop down menu which is highlighted in “Red” in the below image.

Step 6 : In the given page, click on the “Birth/Death Registration Correction” button which is highlighted in “Red” in the below image.

Step 7 : In the new page, Provide the required details and click on “Search” button which is highlighted in “Red” in the below image.

Step 8 : Fill in the required details, upload the required documents and submit the application. Check the details filled for any correction before you submit the form. Submitted forms cannot be altered.

Step 9 : Once the form is submitted, the payment page will open. Make the online payment as instructed. After the payment is successful, an acknowledgement number will be generated. Note it down or take a print out for future reference. The application will be processed further by the concerned department.

Step 10 : Once it is processed, and the updated/corrected death certificate is ready, the applicant will receive an SMS or call on the registered mobile number to collect the certificate. You can also check your application status online.

Step 12 : In the given page, click on the “Application Status/Download Certificate” button which is highlighted in “Red” in the below image.

Step 13 : Fill in the required details and click on the “Search” button which is highlighted in “Red” in the below image to check the application status. Once the certificate is ready, it can be viewed in this page and you can click to download it.
