Certification for DFA, local employment and other purposes
- Here's how you can request a driver's license certification. Where you will apply for the procedure will depend on the reason why you need the document. If you are securing a certification for local purposes, apply at LTO Licensing Centers, District Offices / Extension Offices and License Section, Central Office. For DFA Authentication, apply at License Section, Central Office. Check if you need to book an appointment online with LTO ahead of your visit. Note: LTO just launched a new website portal called the Land Transportation Management System (LTMS) which you can use to set up an appointment and transact with LTO. But not all LTO offices in the country is connected to the system. Some offices may still accept walk in applicants. It is very important that you check first before you go to a LTO office.
- At the LTO office ask where can you obtain a request form.
- Once you have completed the form get a queue ticket and wait for your number to be called.
- Submit the request form together with all the requirements. Don't forget to attach your queue number.
- Proceed to the Cashier to pay the application fee.
- Collect your official receipt (OR) and claim stub.
- You will be informed by the LTO officer when will your Driver's License (DL) certification be ready.
- When you collect your Driver's License (DL) certification, you will be asked to sign the duplicate copy of the certification before the original is released to you.
License Transaction History

- Original and photocopy of the Driver’s License and/or Official Receipt (OR)
- If the original Driver's License is loss, submit affidavit of loss. If the Driver's License holder is out of the country, one (1) photocopy of valid passport showing the first page, visa page and latest date of arrival and departure.
- For the representative, original authorization letter with one (1) photocopy of any valid government/company issued ID with photo and signature of the applicant and representative.
