This procedure explains various ways to Register as Optometrist in Zamboanga City.

1. The first thing you will need to do is check for the schedule of registration which will be posted by PRC on their website.
2. Carefully read the posted announcement. Other than the registration date it will also contain other information like documents you will need to present or submit and how much is the registration fee.
3. Go to a PRC office to obtain an Oath Form or Panunumpa ng Propesyonal.
4. Do not forget to bring the following when you visit the office:
   1. Current Community Tax Certificate (cedula)
   2. One passport size picture (colored with white background and complete nametag)
   3. One piece 1 x 1 picture (colored with white background and complete nametag)
   4. Two sets of metered documentary stamps
   5. One short brown envelope with name and profession
5. Go to the designated counter/window (verification officer) and submit the completed oath form for assessment of prescribed fees.
6. Proceed to the cashier window to pay the prescribed fee. PRC Main Office: Cashiers windows are located at the 3rd, 4th, and 5th floors of the PRC Annex Bldg and/or at the ground floor.
7. Once you have paid the registration fee, proceed to the designated Registration counter/window. The Registration Officer will print the Registry Sheet with assigned Registration / License Number to be signed and thumb marked by the Registrant.
8. After completion of the Registry Sheet, the issuance officer will give you a claim slip/stub that states when you can claim your Professional Identification Card.

For registrants who are currently abroad:

1. Take the professional's oath form before the Philippine Consul.
2. Have a Special Power of Attorney notarized by the Consul.
3. You will need to bring a copy of your passport page showing your personal data and the page showing date of entry abroad.

Following is a procedure on how to apply for the issuance of Professional Identification Card (ID) and Certificate of Registration for Optometrists.

Practice of Optometry: A person performing any of the following acts for a fee (paragraphs 1 to 4) is deemed to be practicing optometry:

1. The examination of the human eye through the employment of subjective and objective procedures, including the use of specific topical diagnostic pharmaceutical agents or drugs and instruments, tools, equipment, implements, visual aids, apparatuses, machines, ocular exercises and related devices, for the purpose of determining the condition and acuity of human vision to correct and improve the same in accordance with subsections b, c and d hereof;
2. The prescription and dispensing of ophthalmic lenses, prisms, contact lenses and their accessories and solutions, frames and their accessories, and supplies for the purpose of correcting and treating defects, deficiencies and abnormalities of vision;
3. The conduct of ocular exercises and vision training, the provision of orthoptics and other devices and procedures to aid and correct abnormalities of human vision, and the installation of prosthetic devices;
4. The counseling of patients with regard to vision and eye care and hygiene;
5. The establishment of offices, clinics, and similar places where optometric services are offered; and
