Apply In-Person:

1. To apply for a death-certificate in Zambia, the applicant will have to contact the Registrar-General office of your area, for the main office visit link.
2. Application for a death certificate can be made in writing by the next of kin of the deceased whose death was registered in Zambia.
3. Make sure that you have all the documents that are listed under the “Required Documents” section of this page.
4. An application form will be provided to the applicant upon visiting the Registrar-General office.
5. Once understanding the guideline properly, the applicant has to fill-in all the necessary information in the application form.
6. Submit the application form with necessary attachments to the office.
7. Pay the fee as per given in the “Fees” section of this page.
8. After the completion and verification of the documents, the certificate will be accessible.
9. The applicant gets the certificate in a stipulated period of time.

• Legal informant to register a death should be: Next of kin, Relative, Funeral undertaker, Responsible government body for unidentified cases.

• Regarding the deceased: Name, Sex, Date of birth or age, Marital status, Place of usual residence
• Regarding the death: Date of death, Place of occurrence of the death, Type of place of occurrence (hospital, home, etc.), Date of registration, Place of registration, Cause of death

• The granted document is used to acquire inheritance.
