Apply In-Person:

1. To donate for a person with disability in Zambia, the applicant will have to visit the agency for persons with disability at the district or provincial office, for the main office visit link.
2. Make sure to have all the documents that are listed under the “Required Documents” section of this page.
3. An application form will be provided to the applicant upon visiting the agency.
4. Once understanding the guideline properly, the applicant has to fill-in all the necessary information in the application form.
5. Submit the application form with necessary attachments to the respective office.
6. There is no fee applicable for this procedure.
7. After the completion and verification of all the documents, the applicant will be able to donate.
8. Complete the donation as per requested by the authorities.
