Apply In-Person

1. To apply for a Pink National Identity Card registration in Zambia, the applicant will have to contact the department of National Registration, Passport and Citizenship office. Please tap on the following link to view contact details link.
2. Make sure to have all the documents that are listed under the “Required Documents” section of this page.
3. Upon visiting, the applicant will be provided with an application form to be filled in the district office. Find the form on page 13 of this pdf file application form.
4. Once understanding the guideline properly, the applicant has to fill in all the necessary information in the application form.
5. Submit the application form with necessary attachments to the office.
6. The Biometric right thumb print, biographic information is also taken during the application process.
7. There is no fee for this process.
8. The application will be duplicated for local and central offices.
9. Upon the verification of all required documents, the ID card will be issued.
10. Collect the Pink National Identity Card from the office in 5 days or wait for it to arrive via post on the provided postal address in a stipulated period of time.

• Completed application form.
• Proof of commonwealth citizenship.
• Passport size photo of the applicant.
• Proof of identity in the form of a birth certificate.
• Or an affidavit signed by a Commissioner of Oaths.
