Wyoming - Obtain Copy of Death Certificate


Procedure

Mail-In RequestEdit

  1. Complete the Application Form.
  2. Submit the request along with the fee of $10.
  3. Money orders or a personalized check from the person making the request should be made payable to Vital Statistics Services.
  4. Please enclose a self-addressed, stamped envelope and application form with the request.

  2. They might charge you a slightly higher fee than the mail-in request.

One of the following Identification document is required.

  • Photocopy of the drivers license.
  • State ID card.
  • Passport of the person requesting the certificate.
  • Notarized signature of the person making the request.
  • If the person named on the certificate doesnt have ID, a parent who has ID may request the certificate.

Eligibility

The person whom you are requesting the Death Certificate for is no longer living and it is been confirmed by a medical practitioner.




Fees

Mail-In RequestEdit

  • fee is $10
  • If the date of death is unknown, a searching fee of $13 for every five years searched is charged, which includes either a certified copy or verification of the record if one is found. If a record is not located, your fee will be retained.

  • fee is approximately $30. But please verify.

Death records are on file since July 1909. Records that are over one hundred (50) years old should be obtained from Wyoming State Archives. Death certificates may be obtained by

  • Immediate family members
  • Lawyers representing the immediate family
  • Bank, executors of estate, insurance company, or anyone requiring a certificate to pay a policy or death benefit on the decedent.
  • If you are not named on the death certificate, please be prepared to prove relationship to the decedent.

  • Date of death
  • City or county of death
  • Relationship to deceased
  • Purpose for which copy is needed
  • Signature of person making request
  • Mailing address where record is to be mailed
  • Identification: Requests must include a photocopy of the drivers license, State ID card, or passport of the person requesting the certificate. We will also accept a notarized signature of the person making the request. The copy should be enlarged to make sure that it is legible.





Need for the Document

Death Certificate document issued by a medical practitioner certifying the deceased state of a person or popularly to a document issued by a person such as a registrar of vital statistics that declares the date, location and cause of a person's death as later entered in an official register of deaths.

In situations involving foreign countries, an apostils may be needed. An apostils is a method of certifying a document for use in another country. With this certification, a document is entitled to recognition in the country of intended use without further action by a consulate or embassy.
