Western Australia - Obtain a Birth Certificate


Procedure

  • Complete the application form at the Registry or print an online application form.
  • You must have an appropriate original identification with you.
  • Submit the completed form at:

  • Certificates will only be issued where:
    • you are eligible to receive the certificate
    • suitable identification is provided
    • fees are paid





Required Documents

  • If you are applying for a certificate or a change of name or a correction, you must provide at least three forms of identification:
    • ONE document from each list (1, 2 and 3). At least one containing a photograph; or
    • ONE from List 1 and TWO from List 2. At least one containing a photograph; or
    • TWO from List 2 and ONE from List 3. At least one containing a signature.
  • All forms of identification produced MUST be current.
  • Documents from List 3 MUST show your current residential address.
  • Bank statements, utility accounts or rates notices MUST have been issued within the last six months.
  • Applying in person - original documents must be provided
  • Applying by post - send clear certified photocopies of your identification documents with your application. Do not post original documents.

Certification requirements

  • Photocopies of identification will only be accepted if they are certified by a qualified person as being "true copies" of the original documents. Information about who can certify documents is attached to the downloadable application form below.

List 1

  • Australian driver's licence (with photo)
  • Australian passport (with photo)
  • Australian firearm's licence (with photo)
  • Defence force/Police ID card (with photo)
  • Department of Immigration and Citizenship (DIAC) certificate with of evidence of residence status (with photo)
  • Over 18 or Proof of Age identity card (with photo)

List 2

  • Debit or Credit card (one or the other, not both) issued by a financial institution
  • Document of Identity issued by the Passport Office
  • Entitlement card issued by the Commonwealth or State Government (Centrelink, Health Care card, Veterans Affairs card etc)
  • Full birth certificate issued in Australia (birth extracts not accepted)
  • Medicare card
  • Naturalisation, citizenship or immigration papers issued by DIAC
  • Overseas passport with current Australian Entry Permit
  • Security guard or crowd control licence (Australian)
  • Student identity document or Statement of enrolment issued by an educational institution, including Tertiary (should include photo and/or signature)
  • Working with children card

List 3

  • Drivers licence renewal notice
  • Financial institution statement less than six months old
  • Motor vehicle registration
  • Property lease or tenancy agreement
  • Shire/water rates notice
  • School or other educational report or certificate less than twelve months old
  • Utility account less than six months old (e.g. gas, electricity, home phone etc)

Applying for certificates on behalf of another person

  • If you are applying for a certificate on behalf of another person, you must provide the following:
  • Identification for yourself;
  • Identification of the person who is entitled to the document as stated in the Registry's Certificate Access policy.

Eligibility

  • A parent named on a child's birth certificate can apply for the certificate. The parent's identification must be provided with the application.
  • A person aged 16 years or more can apply for their birth certificate but identification must be provided.
  • A child under the age of 16 years must have the written consent from a parent named in the certificate and that parents identification must be provided.
  • See the Registry's Birth Certificate Access Policy for full eligibility requirements.
  • If you are not eligible to apply for a certificate you must provide the written consent or authority from an eligible person and you must also provide three forms of identification for yourself and that person (see certification requirements and Letter of Authority download on this page).
  • There is unrestricted access and no identification requirements are needed for births that occurred 100 years ago or more.




Fees

  • Certificates
    • Birth certificate (including a search over any period not exceeding ten years) - $44
  • Additional priority
    • Priority service, in addition to any other fee (not payable for births from 1930 onwards when lodged in person at the Perth Registry Office or Albany, Armadale, Bunbury, Fremantle, Geraldton, Joondalup, Kalgoorlie, Mandurah, Midland or Rockingham Court) - $34
  • Commemorative Birth Certificate
    • Commemorative birth certificate (including the issue of a standard birth certificate) - $54
  • Search fees
    • Search in an index (including a search over any period not exceeding ten years), includes a Result of Search certificate - $31
    • These fees were set 1 July 2009 and subject to change without notice.
  • The following payment methods are accepted at registry:
    • Credit card - MasterCard or Visa only and card number must have 16 digits.
    • Available at the Perth Registry and selected Western Australian courts
    • No other credit cards or travellers cheques will be accepted
    • Eftpos (debit card) when paying in person at the Perth Registry or at selected Western Australian courts.
    • Cheque or money order - made payable to: Registrar of Births, Deaths and Marriages.
    • Note: When paying at a Western Australian court, cheques or money orders should be made payable to the "Department of the Attorney General".
    • Cash - when paying in person at the Perth Registry or at a Western Australian court.
    • Overseas applicants - Must pay in Australian dollars using an international money order or bank draft made payable to: Registrar of Births, Deaths and Marriages.

Instructions

  • An application form can be completed when you apply at the Registry or you can print an online application form. Also take the appropriate original identification with you.
  • There are conditions that the Registrar of Births, Deaths and Marriages must consider when granting access to certificates under Part 8 Division 4 of the Births, Deaths and Marriages Registration Act 1998.
  • In Western Australia, Birth Certificates are available to:
    • the person named in the certificate (16 years of age or over)
  • either parent (named in the certificate)
    • legal guardian (proof of guardianship required)
  • Please refer to the Registry's Birth Certificate Access Policy for a full list of eligible applicants.
  • When applying for a certificate in person three forms of identification must be provided. Photocopies must be certified by a qualified person.
  • When applying by post, three forms of photocopied and certified identification documents must be provided with your application. Do not post original documents.
  • Photocopies of identification will only be accepted if they are certified by a qualified person as being "true copies" of the original documents.
  • The Registry is unable to accept applications by phone, fax, email or online.
  • Note: Some overseas countries require Australian documents to be authenticated. Refer to Smartraveller - Legalising Documents for further information on this requirement.
  • When applying by post, three photocopied and certified identification documents must be provided with your application. Do not post original documents.
  • Photocopies of identification will only be accepted if they are certified by a qualified person as being "true copies" of the original documents.
  • Information about who can certify documents is provided on the application form.
  • Payment is accepted by:
    • Credit card (MasterCard or Visa only)
    • Cheque/money order.
  • Cheques or money orders should be made payable to: Registrar of Births, Deaths & Marriages.
  • If you want your certificate posted by Express Post, an additional charge of $5 is payable for postage within Australia or $13 for international postage.

The Western Australian Registry registers all births that occur in Western Australia since 1 September 1841. If the birth occurred in another State, contact the relevant Registry Office.
