• Submit your new address details via change of address web form.
  
• A change of address sticker will be forwarded when your address details have been changed on your driver's license record. This should be attached to the back of your driver's license document.

Change of address by post or faxEdit

• If you prefer, you can notify the Driver and Vehicle services in writing of your change of address by using the Change of personal details (P64) form provided below.
• The change of address fax number is: 1300 669 995 or from outside Australia +61 89427 8191
• Change of name documents are not acceptable via fax.
• A change of address sticker will be forwarded when your address details have been changed on your driver's license record. This should be attached to the back of your driver's license document.

Anyone in Western Australia who holds a driving license.

There is no fee payable to change your name, address or gender on our system. However a fee will apply if you request a replacement drivers license document showing your new name or address (rather than using the change of address sticker), or wish to change the photograph recorded on your license document.

• It is a legal requirement to notify Driver and Vehicle Services within 21 days of changing your address.

• Full name(new and old details)
• Date of birth
• residential address(old and new)
• postal address
• Full driver license details
• Full vehicle license details
• Boat registration number (if applicable)
• Firearm registration number (if applicable)

If you have changed address, you must need to notify and inform the Department of Transport Driver and Vehicle Services so that your driving license will be updated.
