Western Australia - Amend a Birth Certificate


Procedure

  • Complete the application form at the Registry or print an online application form.
  • You must have an appropriate original identification with you.
  • Submit the completed form to:

Required Documents

  • If you are applying for a certificate or a change of name or a correction, you must provide at least three forms of identification:
    • ONE document from each list (1, 2 and 3). At least one containing a photograph; or
    • ONE from List 1 and TWO from List 2. At least one containing a photograph; or
    • TWO from List 2 and ONE from List 3. At least one containing a signature.
  • All forms of identification produced MUST be current.
  • Documents from List 3 MUST show your current residential address.
  • Bank statements, utility accounts or rates notices MUST have been issued within the last six months.
  • Applying in person - original documents must be provided
  • Applying by post - send clear certified photocopies of your identification documents with your application. Do not post original documents.

Certification requirements

  • Photocopies of identification will only be accepted if they are certified by a qualified person as being "true copies" of the original documents. Information about who can certify documents is attached to the downloadable application form below.

List 1

  • Australian driver's licence (with photo)
  • Australian passport (with photo)
  • Australian firearm's licence (with photo)
  • Defence force/Police ID card (with photo)
  • Department of Immigration and Citizenship (DIAC) certificate with of evidence of residence status (with photo)
  • Over 18 or Proof of Age identity card (with photo)

List 2

  • Debit or Credit card (one or the other, not both) issued by a financial institution
  • Document of Identity issued by the Passport Office
  • Entitlement card issued by the Commonwealth or State Government (Centrelink, Health Care card, Veterans Affairs card etc)
  • Full birth certificate issued in Australia (birth extracts not accepted)
  • Medicare card
  • Naturalisation, citizenship or immigration papers issued by DIAC
  • Overseas passport with current Australian Entry Permit
  • Security guard or crowd control licence (Australian)
  • Student identity document or Statement of enrolment issued by an educational institution, including Tertiary (should include photo and/or signature)
  • Working with children card

List 3

  • Drivers licence renewal notice
  • Financial institution statement less than six months old
  • Motor vehicle registration
  • Property lease or tenancy agreement
  • Shire/water rates notice
  • School or other educational report or certificate less than twelve months old
  • Utility account less than six months old (e.g. gas, electricity, home phone etc)

Applying for certificates on behalf of another person

  • If you are applying for a certificate on behalf of another person, you must provide the following:
  • Identification for yourself;
  • Identification of the person who is entitled to the document as stated in the Registry's Certificate Access policy.

Fees

  • Certificates
    • Birth certificate (including a search over any period not exceeding ten years) - $44
  • Correction of a birth record
    • Addition to, or correction of, information in the register, includes a certificate - $44

Payment Options

  • The following payment methods are accepted at registry:
    • Credit card - MasterCard or Visa only and card number must have 16 digits.
    • Available at the Perth Registry and selected Western Australian courts
    • No other credit cards or travellers cheques will be accepted
    • Eftpos (debit card) when paying in person at the Perth Registry or at selected Western Australian courts.
    • Cheque or money order - made payable to: Registrar of Births, Deaths and Marriages.
    • Note: When paying at a Western Australian court, cheques or money orders should be made payable to the "Department of the Attorney General".
    • Cash - when paying in person at the Perth Registry or at a Western Australian court.
    • Overseas applicants - Must pay in Australian dollars using an international money order or bank draft made payable to: Registrar of Births, Deaths and Marriages.

If all necessary supporting information, including identification documentation, is provided and the required fee is paid, we will amend a record within four days plus postal delivery time.

Instructions

  • If information on a certificate issued by the WA Registry of Births is incorrect or incomplete, you should contact the Registry. Where the error was made by the Registry, no fee is payable to correct the certificate and any certificates issued will be replaced free of charge.
  • However, if incorrect or incomplete information was provided to the Registry, then a correction fee is payable. This fee includes the issue of a replacement certificate. Any request to correct certificate information must be supported by documentary evidence which confirms the correct details.
  • Certificates will only be corrected where:
    • you are an eligible applicant;
    • suitable identification is provided;
    • fees are paid.
  • To correct Registry records, applicants need to supply identification to verify they are entitled to correct the registration.
  • If applying for a certificate in person three forms of your original identification documents or clear photocopied and certified identification documents must be provided.
  • If applying by post, three forms of clear photocopied and certified identification documents must be posted with your application. Do not post original documents.

Adding father's details

  • If your child's birth is registered in Western Australia without the natural father's details, these particulars may be added to the registration by completing the "Add father's details in a child's birth" application form (See download below) and payment of a fee. Both parents must also provide appropriate identification.
  • When adding the natural father's particulars to the birth registration, you have the option of registering your child's family name as:
    • the mother's family name
    • the father's family name
    • a combination of the mother's and father's family name in any order with or without a hyphen; or any other family name.
  • Note: The Registry can only add the particulars of a child's natural father.

Adding marriage details

  • If your child's birth is registered in Western Australia and the natural parents have married after the child's birth, the marriage particulars, and if not already recorded, the father's details may be added by completing an application form "Add marriage details in a child's birth" or "Add father's and marriage details to a child's birth" (See downloads below) and payment of a fee. Both parents must also provide appropriate identification.
  • When adding these particulars to the birth registration, you have the option of registering your child's family name as:
    • the mother's family name
    • the father's family name
    • a combination of the mother's and father's family name in any order with or without a hyphen; or any other family name.
  • Note: The Registry can only add the particulars of a child's natural father and the details of his marriage to the mother.
  • If your child was born interstate, contact the Registry Office in the State or Territory where your child was born.

Adding same sex parent's details

  • If you are in a same sex relationship and you would like to add your partner's name (partner who consented to the Artificial Fertilisation procedure) to your childs birth registration, contact the Registry for the appropriate form. Both parents must also provide appropriate identification.
  • Note: If the child was born interstate, contact the Registry Office in the State or Territory where the child was born.

A birth certificate provides legal evidence of your name, age, place of birth and parents details. A birth certificate is a primary identity document. You need a birth certificate to enroll a child in school, join a sporting club, to get a passport or drivers licence, gain employment or government benefits and to get married.
