1. Ration Card holder should approach the ration office of your area ORThe Inspector Food & Supplies as applicable.

3. Please go to the respective office.
4. Applicant shall obtain the form from the respective office (please pay for the form if required) Or a written/electronically typed application on A4 plain sheet as advised shall be used or use the following link to download link for form
5. Download Form-3, choose from Urban or Rural according to your area of residence.
6. Please submit the completed application along with the required documents to the respective authority.
7. Authorities after checking the submitted application along with the attached documents will make entries and provide you record number for reference.
8. Keep it safe for future reference.
9. This application will be forwarded to the concerned Ward/ GramaPanchayath official for verification. The concerned official visits the house of the applicant for verification and approves the request, if the applicant is found eligible.
10. The applicant is informed through SMS at various stages i.e., Advance intimation about date of visit of the official to his place of residence for verification of his application, Approval/Rejection of his application.
11. After ground level verification ration card will be issued in 2 – 3 weeks as notified by the department.

• A valid ration card holder.
• The applicant and his/her family must be Indian citizens and should be resident of the state.
• The applicant or his family members must not possess any family card in any State in India (If they had one, it has to be withdrawn).
• The applicant and members of the family must be close relatives.
• New born above 5 years
• Addition of member name after marriage

• Fee information can be obtained directly from the department office while applying.

• Validity remains same
• Usually Family identity card is valid for lifetime and will be renewed in 5 years or based on announcement.
