1. That declaration form has to be certified by the ward or commune police and her sealed photos. (4 photos)
2. Attaching the photocopy of Identification Card of the person doing the procedure to apply for passports for children under 14 years.

• The applicant can call 1080 (The Information Center) to ask the address of the Immigration Department near where you live most and come there to apply for passport documents.
• Completed 1 declaration form with 4 passport photos
• In case of necessity the applicants must bring original household registration book or its endorsed copy to collate.
• When applying for passport, the applicants must present the original ID to check.

Every child regardless of age traveling overseas must have a valid passport

Passport fee: VND 200,000

Passports for children under 14 years old (issued separately or issued with the passport of mother or father) with a term of 2 years to 5 years from the date of issuance and is not extended. In case of adding children less than 14 years old on the issued passport of mother or father, the validity is ??from 2 years to 5 years from the date when children was added.

• Fill up 1 declaration form in the prescribed form. That declaration form is not certified by the Ward or Commune Police in your permanent residence or temporary residence.
• Passport application for Vietnamese citizens has to apply at the Provincial Immigration Department.
• Submit additional documents as the following cases:
  • In case of including in their parents passports, there must be 1 photocopy of birth certificate (bring the original for comparison).
  • In case of applying for separately child passport, there must be 1 above declaration form which is declared and signed by their parents and 1 photocopy of birth certificate (bring the original for comparison).
  • In case there are no longer parents, their adoptive parents or legal guardians (with papers proving that) can declare and sign the declaration form.

• Full name
• Date of birth
• Address
• Full name of parents, adoptive parents or legal guardians.

Passports for children can be included in the passports of their parents, legal guardians or issued separately for the child.
