2. Read the application form carefully.
3. Fill in the front page (Page 1) and sign the signature boxes inside the form (on Page 2).
4. Get two identical colour Passport photos. Have them endorsed by someone appropriate (see inside form or Page 2+3).
5. Provide original proof of your date of birth, your identity and your address (Page 4).
6. Send or attend our office with the completed form and signed photos, the original supporting documents and payment.
