Use this form to order a marriage certificate from the Department of Health for marriages that occurred January 1, 2010 or LATER - Marriage Certificate Order Form

  1. Fill out and print the request form.
  2. Sign and date the application.
  3. Make check or money order payable to "Vermont Department of Health". Note: Do NOT mail cash
  4. Send the completed form along with your payment to:

Vermont State Archives and Records Administration (VSARA)Edit

Certified copies of vital records cost $10.00 per certification and may be requested online through VSARA's Vital Records Request Service, through U.S. Mail, or in person. Authenticating a certificate for foreign use is an additional $10.00.

Vermont State Archives and Records Administration (VSARA): Request for certificates, whether received online or through the mail, are processed within a week. Expedited shipping of records is also available at an additional charge. Expedited orders will be shipped overnight on the day your order is fulfilled.

Vital Records Office

Since 2000, the Vermont vital records system includes eight types of vital events: births, deaths, marriages, divorces, civil unions, dissolutions, fetal deaths, and abortions.

Records for events that occurred within the past five years are maintained by the Department of Health. All others are with the Vermont State Archives and Records Administration.


Which office you must contact to order a vital record depends on how long ago the event took place:

  • New records - occurred in 2010 through the present
Order from the Department of Health
  • Older records - occurred in 2009 and earlier
Order from the Vermont State Archives and Records Administration

Need for the Document

A marriage certificate is the official record of two people joining together in a marriage. This certificate is required to make the union between two people legally binding. If you have lost your marriage certificate or if you simply want a duplicate copy of your marriage certificate you can obtain one by following the procedures given below.
