   Note: The applicant can contact the Residential Construction Branch for completing the survey at: 1–800–845–8244. Monday through Friday, 8:00 a.m. to 5:00 p.m. Eastern time.

4. For completing this also refer the sample survey form which is mentioned in the “Sample Documents” section of this page.
5. Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
6. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
7. Submit all the documents with the completed form to the office mail address.
8. The mailing address is mentioned in the “Office Locations & Contacts” section of this page.
9. Once the verification process gets completed, the applicant gets the notification from the officer through mail or SMS.
10. If the officer is not satisfied with the verification process, the respective officer will come for inspection.
11. The fee for this procedure has to be paid as per the authority’s request.
12. After the process gets completed, the applicant gets the permit through mail.
13. The processing time for this procedure is up to 30 days.

• The monthly statistics data is based on sample of 8,500 permit-issuing places, selected from a universe of approximately 19,000 places.
• The annual statistics are not based upon a sample, but its obtained by direct cumulating the data for the 19,000 permit-issuing places.
