1. Gather all documents you will need to present or submit to apply for the procedure.
2. Fill out an application form.
3. The completed form must be submitted personally by the applicant along with all other required documents.
4. For applications submitted in Uzbekistan, it will take about 10 working days for the process to be completed, 15 working days for application submitted abroad.

To apply for a passport for the first time:

• Copy of birth certificate
• Copies of passports of parents sheets on which there are records or notes . In case the applicant can not provide a copy of the passport of their parent, the applicant may attach a copy of the following documents: death certificates , divorce certificate.
• Completed application form
• Photo of the applicant - 4 pcs . , Sized 35 x 45 mm
• A certified copy of a piece of departure , which is inscribed in the applicant ( for CIS countries)
• If one parent is a foreign national: applicant must provide a legalized (for CIS notarized) statement of the foreign parent's for to submit an application to obtain a child passport issued by the Government of Uzbekistan
• A copy of the registration card to the applicant's permanent consulate

For damaged passports or in case there no more available pages on the passport for visas and special marks:

• A copy of the passport pages on which there are records or mark
• Completed application form
• Photo of the applicant - 4 pcs . , Sized 35 x 45 mm
• Document confirming leaving to relocate - the presence in the passport sticker issued by MIA RU and for CIS stamp extracts from addresses in the Republic of Uzbekistan and the copy sheet departure

For lost or stolen passports:

• Copy of passport (if available)
• Completed application form
• Photo of the applicant - 4 pcs . , Sized 35 x 45 mm
• Statement detailing the circumstances of loss of passport
• A completed application form to request the applicant to verify identity
• A document issued by the law enforcement authorities of the host country upon the request of the applicant containing details about the loss of the passport

To apply for change of name:

• A copy of the passport pages on which there are records or mark;
• Completed application form
• Photo of the applicant - 4 pcs . , Sized 35 x 45 mm
• Document confirming leaving to relocate - the presence in the passport sticker issued by MIA RU and for CIS stamp extracts from addresses in the Republic of Uzbekistan and the copy sheet departure

Note: The passport photos must be printed on quality photo paper. The photo must be taken from the front, without a hat, with the exception of clothing with a high neckline or cleavage.

A passport is a document, issued by the national government. It certifies the identity and nationality of the holder for travel abroad. It contains information about the holder; like name, gender, date and place of birth.

This document does not in itself entitle the passport holder entry into another country, nor consular protection while abroad nor any other privileges. However, it does normally allows the passport holder to return to the country where the passport was issued.

Rights to consular protection arises from international treaties, while the holder's return to the issuing country depends on the laws prevailing in that country.
