1. Applicants must complete a driver education course.
2. Go to a driver license office.
3. Complete a drivers license application, to save you some time you can:
   1. Fill out and print the application ahead of time online and bring it with you when you go to the office, or
   2. You can schedule an appointment prior to your visit to avoid waiting in line.
4. Have your photo taken.
5. Present your learner permit.
6. Pass the eye test.
7. Complete the medical questionnaire.
8. Have your parent/legal guardian sign off on 40 hours of practice driving (10 hours at night); Parent/legal guardian sign for financial responsibility.
9. Pay the nonrefundable fee of $30.00.

Driving Test

You have three options to complete your driving test.

• You can schedule a driving test appointment ahead of time by calling the driver license office you plan to visit to complete your license.
• You can complete the test through your driver education class at your high school, or
• You can also contact a certified commercial school and pay a fee to be tested

To those who are new to Utah and have been previously licensed in another U.S. State or Country, you are required to:

1. Go to a driver license office.

5. Applicants will also need to provide proof of a valid U.S. State or country license. If coming from another U.S. State your license will be surrendered upon application and a validation check will be ran by the examiner to be sure your status is valid in all other states;
6. Pass the eye test.
7. Complete the medical questionnaire.
8. Pass a 25 question open-book test; and
9. Pay the nonrefundable fee of $30.00.

After you have met all the requirements needed to obtain a driver's license, you will receive a temporary photo license on the same day and the completed license will be mailed to you in approximately three weeks.

• Applicants will need to submit driver education, or a valid license from another state (which you will surrender upon application), or country to avoid having to carry a learner permit for 90 days.
• Applicants will also need to submit proof of birth, full social security number and residence.

At any time the parent or legal guardian who signed for financial responsibility on the minor's application can choose to withdraw their signature until the minor is 18 years of age, and for any reason. This action would invalidate the driver license and the minor could reapply when they reach the age of 18, or if the parent/legal guardian signed a new application. There would be a licensing fee and written test to reapply.
