Apply In Person:

1. To change a Name (Deed Poll), the applicant has to approach the local Federal courts link
2. The applicant has to fill out the form with the necessary details to change the name.
3. The applicant has to sign in the affidavit that he/she is not a convicted felon or has a current outstanding warrant.

   Note: Don’t sign the form until the applicant brings it to a notary public who will verify the identity and witness the applicant's signature.

4. Submit the completed form with the required documents to the court clerk.
5. Make sure that the applicant should have all the documents mentioned in the “Required Documents” section of this page.
6. The fee for this procedure has to be paid as per the authority's request.
7. The clerk will then date stamp the petition and enter it into the public records and return a copy to the applicant.
8. Fill out a proposed order for the judge /magistrate to sign then the clerk will schedule you for a hearing.
9. The judge/magistrate presiding the case may ask you a question like reasons for changing your name, and if the is no objection the judge will issue the certificate.
10. The processing time for this procedure takes up to one year.
