1. Complete an application and submit it together with these following documents to Health Authority - Abu Dhabi's Customer Service Centre:
   1. Family book (for UAE Nationals) : Original and Copy
   2. Emirates ID Card : Original and copy
   3. Parents passport : Original and copy
   4. Child's Passport : Original and copy
   5. Birth Certificate: Copy, if any
2. Pay the required fee.
3. Receive the replacement for the lost birth certificate.

You may use the link provided under "Office Locations & Contacts" to get in touch with the nearest UAE Mission in the country you are currently staying. They may be able to help you submit your request to obtain a certified copy of a birth certificate

The applicant should be one of the parents or relatives up to the fourth class, if the applicant is not one of the parents or relatives, he/she should present a personal identification (passport or Emirates ID) or an official authorisation from the court allowing him/ her to apply for and receive the birth certificate.
