1. To request for a duplicate certificate of registration of trademark you will need to complete a request form and submit it to the Intellectual Property Office.
2. A link is provided under 'Documents to Use' section which you can use to download the form. The form is available in PDF format.You will need Adobe Acrobat® (pdf reader) in order to view the downloaded form. All forms must be printed on A4 paper.
3. Submit the completed form and pay the application fee.

A trade mark is valid for ten years from the filing date of the application and may be renewed for successive ten year periods upon the filing of the prescribed renewal application form and the payment of the prescribed fee.
