1. Duplicates of a certificate may be issued in case of loss, theft or destruction. They may also be issued in case of deterioration. To obtain a certified copy of a death certificate, you will need to go to your local district office.
2. Do not forget to bring a valid identification. The registry personnel will ask for your ID in order to confirm your identity and make sure that you have the right to obtain the certificate. Note: The applicant's relationship to the deceased must be as a household member, such as a son, daughter, parent, spouse, or close relative.
3. When you submit your request make sure that you have provided these important details which will make searching for the record easier:
   1. Name of the deceased
   2. Sex
   3. Date of death
4. Pay the application fee and wait for instructions from the Registrar regarding when can you comeback to collect the certificate.
