2. Present your current license and the original or certified document showing the reason for the change.
3. Complete the Change of Name form.
4. Pay the fee.

• You must go in person to the Driver Service Center when you need to change your name on your license.
• You will be charged for a new license.

• Name
• Date and place of birth
• Driver license number

Notification to the proper authorities should be send out once the change of your name is already finalized. In that way, your new name will be reflected in your licenses or ID.
