Telangana - Obtain a Family Member Certificate


Procedure

Apply In-Person:

  1. To obtain a family member certificate, applicant has to visit the Tahsildar / Mandal office of their area.
  2. Click the following link to know the tahsildar’s contact of Hyderabad district Hyderabad district Tahsildar Contact.
  3. Approach the respective authority in the tahsildar office and obtain the application form.
  4. Also you can download the application form from the following link: Meeseva Forms. Please scroll down and click on “Revenue” department. Then select “FAMCGOVT Employees (family member)” option to download the application form.
  5. Complete the application form with valid details.
  6. Please make sure that you have all the required documents mentioned in the “Required Documents” section below.
  7. Attach all the mandatory documents with the application form and submit it to the respective officer.
  8. Please pay the fee as disclosed by the authority, if applicable.
  9. Now verification process will be carried out by MRO of that respective district.
  10. Once all the documents and details are verified, applicant will be notified to collect the family member certificate from the tahsildar office.

Apply through Meeseva centers:

  1. Applicant shall visit the nearest Meeseva center and apply for family member certificate.
  2. To know the Authorized service provider contact details visit the following link Meeseva centre details.
  3. Click on the “MeeSeva Centres” option in the menu and select the “Map View” option.
  4. Please make sure that you have all the required documents in hand as mentioned in the “Required Documents” section below.
  5. Download the application form from the following link: Meeseva Forms. Please scroll down and click on “Revenue” department. Then select “FAMCGOVT Employees (family member)” option to download the application form.
  6. Applicant has to submit the required documents along with the filled application form to the Meeseva operator.
  7. Meeseva operator will process the application. On completion of online filing, application number will be provided to applicant for future reference.
  8. Fee amount shall be paid to the operator.
  9. Now verification process will be carried out by MRO of that respective district.
  10. Once all the documents and details are verified, applicant will be notified to download and collect the family member certificate from the meeseva center operator.

Apply Online:

  1. To apply for family member certificate through online, visit the following link: Apply Online.
  2. Please select "CITIZEN" option from the dropdown menu. Now click “New User” option to register into the portal and apply for the services.
  3. Now complete the registration form and click “Submit” tab.
  4. Now enter the OTP in the textbox and click “Submit” tab.
  5. Now click “Home” tab to go back to the home page.
  6. In the home page please click “Login” option at the left side of the page.
  7. Now choose the option as “CITIZEN” from the dropdown menu before login. Then enter valid credentials and click “Login” tab.
  8. In the search box type as “family”. Now click “FAMILY MEMBERSHIP CERTIFICATE” option to apply.
  9. Complete the application form with valid information. To add new member click “Add New Member” tab.
  10. Use “Choose File” tab to attach documents. Then click “Show Payment” tab to proceed further.
  11. Note down the user charges and click “Confirm Payment” tab to go to the payment page.
  12. Choose any one of the payment option and click “Make Payment” tab to complete the payment process.
  13. Now take a printout of the receipt and application form.
  14. Visit the nearest meeseva center and submit the completed application form along with the receipt. Then collect the acknowledgment from the authority.
  15. After verification process, applicant shall be notified to download the certificate from the portal.

  • EPIC Card
  • Identity proof like Ration card, Voter ID, Driving license etc
  • Notarized affidavit containing the name, sex, age, relationship with deceased.
  • Death certificate
  • Additional proof like Passport, Bank passbook indicating the relationship with the deceased.

Eligibility

  • Applicant should be the citizen of India.
  • Applicant shall be the member of that family to whom this certificate is required.
  • Applicant shall hold a valid ration card.
  • The person with whom relationship is being established should be a government employee or Ex-employee. For non-government employee, need to visit court.

Fees

Fee of Rs. 45 is charged.


Validity

Family member certificate is valid forever.

Required Information

The following information is required to complete the application form:

  • Full Name of the applicant
  • Other family member names (who are alive)
  • Applicants Date of birth
  • Relationship to the applicant
  • Every family members date of birth
  • Every applicant’s occupation
  • Passport Size photographs of the applicant
  • Gazetted officer should duly attest the application form
  • Gazetted officer should duly sign on the bond paper with the proper proforma
  • Xerox copy of the family head death certificate
  • Xerox copy of the completed application


Need for the Document

  • If the head or a member of the family expires, the next direct Family member of the deceased such as the wife or husband or son or daughter or mother may apply for family member / Proper person certificate for the purpose of transferring Electricity connection, House Tax, Telephone connection/patta transfer, Bank Account, etc.
  • If the person who died is a Government servant, Family Member Certificate is issued for sanction of family pension, and for getting appointment on compassionate grounds.
  • To establish relationship for claims relating to Insurance, pension, retirement benefits or service benefits of central and state government departments, Government undertakings etc.
