Apply In-Person:

1. Application form for replacing lost birth certificate is available in Registration, Insolvency and Trusteeship Agency contact link
2. Applicant must provide all information that are requested in the application form.
3. Applicant should make sure to carry both photocopies and originals of all documents that are needed for completing this procedure.
4. Documents that are needed for completing this procedure is mentioned under “Required Documents” section of this page.
5. Applicant should attach photocopies of documents along with the application form while submitting it at the office.
6. Fee for this procedure has to be paid as per authority’s request.
7. If all the above-mentioned procedures were followed properly by the applicant, the renewal can be done within 1 week.
8. Applicant will be notified regarding the availability of renewal documents by concerned authorities.
9. Applicant can visit the office after getting the notification for collecting their renewed birth certificate.
10. If there is no information regarding the license is provided, applicant can visit the office to know status of their application.
