Tanzania - Obtain a Death Certificate

Apply In-Person:

  1. To Obtain a Death Certificate, the applicant has to visit the Registration, Insolvency, and Trusteeship Agency (RITA) contact link
  2. Downloadable application form is available in the following link
  3. Applicant must provide all information that is requested in the application form.
  4. Applicant should make sure to carry all documents (both photocopies and originals) of all documents that are needed for completing this procedure.
  5. Documents that are needed for completing this procedure are mentioned under “Required Documents” section of this page.
  6. Applicant should attach both photocopies and originals of all documents (whichever is necessary) along with the application form before submitting it at the office.
  7. Fee for this procedure has to be paid as per the authority’s request.
  8. If all these procedures were followed properly by the applicant, the certificate will be provided within a week.
  9. If there is no information regarding the death certificate is provided within a week, the applicant can visit the office to know status of their application.

  • ID Proof of the Deceased
  • Funeral Permit
  • Passport sized photograph – 1 (of the applicant)
  • Document from the doctor confirming death
  • Confirmation letter from the heirs or relatives (who were there at the time of death)
