Apply In-Person:

1. Application form for obtaining Plant Inspection Certificate is available in Occupational Safety and Health Authority contact link
2. Applicant has to provide all information in the application form.
3. Applicant should make sure to carry both photocopies and originals of all documents that are needed for completing this procedure.
4. Documents that are needed for completing this procedure is mentioned under “Required Documents” section of this page.
5. Applicant must attach photocopies of all documents (whichever is necessary) along with the application form.
6. Fee for this procedure is TZS 6,250,000/- that has to be paid along with the application form.
7. If all the above-mentioned procedures were followed properly by the applicant, they will be notified regarding the certificate availability within 10 days.
8. Applicant can visit the office for getting their license after getting the notification from concerned authorities.
9. If there is no notification provided by concerned authorities, the applicant can visit the office to know status of their application.
