Apply In-Person:

1. To Obtain the Duplicate Registration Certificate of Imported Medical Devices, the applicant has to visit the Tanzania Medical Devices Authority Link
2. Applicantsmust obtain the application form from the office and fill it up with all the necessary information in it.
3. Documents that are needed for completing this procedure are mentioned under the “Required Documents” section of this page.
4. Applicants should attach photocopies of needed documents along with the application form while submitting it at the office.
5. Fee for this procedure has to be provided as per the authority’s request.
6. Once the fee is paid, the receipt for the payment will be provided by concerned authorities, the receipt has to be kept safe by the applicant for future use.
7. The processing time for this procedure will be mentioned by the concerned authorities.
8. Applicant will get a notification via SMS or E-mail regarding the availability of the document, right after receiving the notification, the applicant can visit the office and get their document.
9. If there is no information regarding the license that needs to be provided, the applicant can visit the office and know the status of their application.
