3. Applicant must provide all information that is requested in the application form.
4. Applicant should carry both photocopies and originals of all the documents that are needed for completing this procedure.
5. Documents that are needed for completing this procedure are mentioned under the “Required Documents” section of this page.
6. Applicant should attach all required documents (both photocopies and originals, whichever is necessary) along with the application form before submitting them at the office.
7. Fee for this procedure can be paid as per authority’s request.
8. If all these procedures were followed properly by the applicant, the national health insurance fund will be provided within a stipulated period of time.
9. If there is no information regarding the document is provided, the applicant can visit the office to know status of their application.

1. Online application for applying to National Health Insurance Fund is available in the following online link
2. If the applicant is a registered user, they can login directly using their “Username”, “Password”, and by clicking on “Sign In” option.
3. If the applicant is a new user, they have to register first on the website.
4. Once after logging in, the applicant should provide all the information that is requested in the online application form.
5. The applicant should upload digital copies of all the documents that are requested in the website.
6. The Fee for this procedure has to be paid as per the authority’s request.
7. Finally, the applicant should click on “Submit” button for submitting the application form.
8. The applicant will be notified regarding the availability of the document on their email or SMS.
9. If there is no information regarding the document is provided, the applicant can visit the office to know the status of the application.
