1. Application form for CPD Course Accreditation is available in The Medical Council of Tanzania contact link
2. Applicant must provide all information that is requested in the application form.
3. Applicant should make sure to carry photocopies and originals of all documents that are needed for completing this procedure.
4. Documents that are needed for completing this procedure are mentioned under “Required Documents” section of this page.
5. Applicant must attach photocopies of all documents (whichever is necessary) along with the application form while submitting it at the office.
6. Fee for this procedure has to be paid as per authority’s request.
7. If all these procedures were followed properly, accreditation will be provided within stipulated period of time.

• Curriculum Vitae (CV) of the applicant
• Professional Certificate that is approved by government of Tanzania
• Company’s profile
• List and phots of training equipment and facilities
• Instructional Design – Practical/Teaching sessions, Telemedicines, Workshop etc…
• Annual plan for proposed CPD activities

• Fee for this procedure has to be paid as per authority’s request.

• CPD Course Accreditation is valid for 1 year.
