1. To obtain a return certificate, applicants will need to go the nearest embassy of Tajikistan.
2. Applicants will need to complete an application. You may use the link provided under 'Documents to Use' section to download the form.
3. The completed form must be submitted together with these documents:
   1. A document issued by the Department of the Interior or other law enforcement authorities of the host country of the Republic of Tajikistan citizen regarding referral of the citizen over the loss of a passport
   2. A document proving the identity and affiliation of the applicant to the citizenship of the Republic of Tajikistan (passport of the citizen of the Republic of Tajikistan, a birth certificate, military ID, ID soldier card, military reserve officer ticket), or certified by a consular officer in the prescribed manner a written statement at least two people the Republic of Tajikistan to support the applicant belonging to the citizenship of the Republic of Tajikistan, with copies of their passports.
   3. Two (2) colored photos 34
