1. You can request for copies of a death certificate in person at the Civil Registry office. Make sure that you bring a valid identification in case the registry officer will need to verify your identity and right to obtain the certificate.
2. You may use the contact information given below to inquire where can you submit your application. When you submit your request at the Registry Office make sure that you have provided these important details which will make searching for the record easier:
   1. names and surnames of the deceased
   2. place and date of birth
   3. place and date of death
3. Pay for the certificate.
4. You will be informed when can you come back to collect the Death Certificate.
