A death certificate is an official document issued by the government, which declares cause of death, location of death, time of death and some other personal information about the deceased.
HOW TO APPLY
- Visit the Divisional Secretariat of the area where the death occurred.
- The application form can be obtained from any Divisional Secretariat office
- Fees chargeable
- Where the date of registration or the no. of the entry is given the fee for one copy of the certificate is Rs. 100.00
- Where the date of registration or the no. of the entry is not given and a search of registers not exceeding two years is involved fee for one copy of the certificate is Rs. 200.00
- Required fee should pay to the Divisional Secretariat.
- Duly filled application click hereshould submit to the Divisional Secretariat.
- If you like to obtain the certificate of death by post, submit a stamped envelope with the application click here.
- If the death is registered a certificate of death will be send and if the death is not registered a letter to the effect will be send.
Required Documents
- Passport and other available identification documents of the deceased, to prove Sri Lankan Citizenship
- Medical report and signature from a physician or coroner to validate the cause of death

Required Information
- Medical report and signature from a physician or coroner i.e. to determine cause of death
- Diseased identification documents i.e. identification card or passport
Need for the Document
