1. You can claim an extract of birth for yourself in person at the Registry Office. If you are obtaining the certificate for another person, you need his/her written authorisation. Note: The authorisation must contain information about the authorising person as well as information on the authorised person and authorising person's signature.
2. You may use the contact information given below to inquire where can you submit your application. When you submit your request at the Registry Office make sure that you have provided these important details which will make searching for the record easier:
   1. name of the person whose birth certificate you are requesting
   2. place and date of birth
   3. parents name
3. Pay for the certificate.

In the central register of births, all births of Slovenian citizens in the Republic of Slovenia or abroad and all births of foreign citizens in the territory of the Republic of Slovenia are entered.

The extract contains:

• unique personal identification number;
• sex;
• citizenship;
• data on parents.
