• Fill in all sections of the birth certificate application form.

• Pay the return postage fee (if required)

For service by mail, fax or email to the Slovak Registry offices. The Birth Certificate Application must be signed and dated in front of a notary public. Payment is accepted by credit card (MasterCard, VISA, or Discover), check or money order and is deposited upon receipt.

Birth certificates can be obtained by;

• The person named on the birth certificate.
• A parent of the person named on the birth certificate (requesting parent's name must be on birth certificate).
• Only by order of a Slovak Court may a spouse, child or other persons obtain a copy of a birth certificate.

Original birth certificate is issued free of charge, for a duplicate to be paid 5.

The registration of births and issuing of birth certificates in the Slovak Republic is regulated by the Act No. 154/1994 Coll. on Civil Registers.

Local People's Committees in many cities, including Horna Suca, District Trencin are authorized to issue birth certificates, marriage certificates, and similar civil registration documents in Bratislava, Piestany, Kosice,or in other cities.

Note : Apostilles are only issued for birth certificates bearing the name and signature of a current official. If the official who signed the birth certificate for which you want to obtain an apostille is no longer in office you will be asked to first obtain a new birth certificate.

• Day, month, year and place of birth of the child; in case of multiple births also the time order of birth,
• Name, surname, gender and birth identity number of the child,
• Name, surname, maiden name, day, month, year and place of birth, birth identity number, citizenship and permanent residence of parents of the child,
• Agreement between parents on the surname of the child, if their surnames are different or in case they are not married; alternatively the courts decision on the surname of the child,
• Day, month and year of the entry

• the full certificate. This is a copy of the entry in the birth register, giving all the recorded details
• the short certificate. This only gives the childs full name, sex, date and place of birth. It does not give the name and particulars of the mother or father. A short certificate is issued free of charge when a birth is first registered
