Application for issue of a passport by the child's parent, accompanied by those of the child's birth certificate and his identity card or passport (if not the holder of identity card). It is vital to charge an administrative fee for issuing the passport.
Both parents must sign the Passport Application Form. Where a father has applied to the court to become a joint guardian and the case has not yet been heard, his consent may be required.
If a joint guardian refuses to sign the Passport Application Form then the parent/guardian applying for the passport may apply to the local District Court to have the other guardian's consent dispensed with.

The Ministry of Foreign Affairs of the Slovak Republic recommends the parents who plan to travel outside of the Slovak Republic with their children after June 26, 2012 to have individual passports issued for their children.
Passport person 6-16 years - 13 to 30 days, 26 to 10 working days and 39 within 2 working days.
Passport person younger than 6 years - 8 to 30 days, 16 to 10 working days and 24 within 2 working days.

A Child Passport is an official document issued by a government, certifying a childs identity and citizenship and entitling them to travel under its protection to and from foreign countries.
A passport is a public document, which the citizens to prove their identity, nationality and other facts contained in the data in the travel document. A passport is the property of the Slovak Republic.
If travelling from one border-free Schengen country to another however, EU nationals are not required to show a passport or national ID card. It is still recommended that you travel with your passport or ID card to prove your identity if necessary though. Note that Bulgaria, Croatia, Cyprus, Ireland, Romania and the UK are not part of the Schengen area, so a passport or ID card is required if travelling to/from these countries.

Slovak citizens might encounter serious problems mainly when crossing external borders of the Schengen Area in some other member states if their children, even younger than five, do not have their own travel documents.
